N.Y. Banking Law Section 4003
Organization certificate to be submitted to the superintendent

The organization certificate of a bank, trust company, stock-form savings bank, stock-form savings and loan association, safe deposit company or investment company, executed in duplicate, shall be submitted to the superintendent at his office.
