N.Y. Public Health Law Section 4163
Penalties

Any person who shall release information which might disclose the identity of the pregnant person in connection with a certificate of pregnancy loss or report of pregnancy loss in violation of the provisions of this title shall be subject to a civil penalty not to exceed five thousand dollars for each such release. Such penalty may be recovered in the same manner as the penalty provided in § 12 (Violations of health laws or regulations)section twelve of this chapter.
